National Task Force Discusses testing, treatment and surveillance strategies for COVID-19 in view of the new virus strain from UK

Posted On: 26 DEC 2020 5:59PM by PIB Delhi

A meeting of the National Task Force (NTF) on COVID-19 was convened today by ICMR under co-chairpersonship of Prof. Vinod Paul, Member Niti Aayog and Prof. Balram Bhargava, Secretary, Deptt. of Health Research & Director General ICMR. The meeting was also attended by Prof. Randeep Guleria, Director, AIIMS; Director General Health Services (DGHS); Drug Controller General of India (DCGI); Director, National Center for Disease Control (NCDC); other representatives from Ministry of Health and ICMR as well as independent subject experts.

Main objective of the NTF was to discuss evidence-based modifications in testing, treatment and surveillance strategies for SARS-CoV-2 in view of the recent reports of emergence of a new variant strain of the virus from UK. The variant strain has 14 non-synonymous (amino acid altering) mutations, 6 synonymous (non amino-acid altering), and 3 deletions. Eight mutations are present in the Spike (S) gene which carries the binding site (Receptor Binding Domain) of the ACE2 receptors, which are the point of entry of the virus into the human respiratory cells.

NTF deliberated in detail on aspects related to current National Treatment Protocol, testing strategy and surveillance of SARS-CoV-2 vis-à-vis the UK variant strain. It was emphasized that since the UK variant strain is implicated to cause increased transmissibility of the virus, it is critical to identify individuals infected with this strain and adequately contain them to prevent its transmission in India.

NTF concluded that there is no need to change the existing Treatment Protocol in view of mutations emerging in the strain. Further, since ICMR has always advocated use of two or more gene assays for testing SARS-CoV-2, it is unlikely to miss infected cases using the current testing strategy.

NTF recommended that in addition to the existing surveillance strategies, it is critical to conduct enhanced genomic surveillance for SARS-CoV-2 specially in incoming passengers from UK. Besides, it will also be critical to conduct genome sequencing in samples where there is dropout of the S gene in lab diagnosis, proven cases of re-infections etc. Routine genomic surveillance of SARS-CoV-2 from representative samples all across the samples needs to be a continuous and well planned activity.

NCDC informed that Government of India has taken cognizance of the reports of mutant variant of SARS-CoV-2 reported from UK and the response of other Countries to these reports. The situation is being monitored proactively. A strategy has been put under place to detect & contain the mutant variant.

The salient features of the strategy are:

A. At points of entry (All International Airports in India):

* All passengers who had arrived from UK between 21st December - 23rd December 2020 have been tested at the Airports

* Only after the RT-PCR test result are available, the negative passengers are permitted to exit the airports

* All positively tested passengers are put under institutional isolation and their samples sent for Whole Genome Sequencing (WGS)

* Only after the confirmation of the non-mutant variant upon WGS result, the positive cases are permitted to leave institutional isolation as per the existing management protocol

* All the contacts of the positive cases are also put under facility quarantine and are tested as per the ICMR guidelines

B.  Community Surveillance:

* List of all UK arrivals during the last 28 days has been shared by Bureau of Immigration with the concerned States

* All passengers who had arrived from UK between 25th November - 20th December 2020 are being tracked by the IDSP State Surveillance Units (SSUs) and District Surveillance Units (DSUs)

* Testing as per the ICMR guidelines of these passengers is being pursued and all positive cases are put under mandatory facility isolation

* Samples of all positive cases are being sent for WGS

* Enhanced contact tracing of these positive cases is being undertaken and these contacts are also put under facility quarantine

* Discharge of positive cases only after ensuring two samples testing negative after 14 days

C. Prospective surveillance:

* 5% of the positive cases from all States/UTs will be tested for WGS

* A genomic surveillance consortium, INSACOG, has been formed under the leadership of NCDC, New Delhi for laboratory and epidemiological surveillance of circulating strains of SARS-CoV-2 in the Country.Further, more than 50 samples of UK returnees are currently under sequencing at the designated laboratories. Other laboratories in the consortium are: National Centre for Disease Control, Delhi; CSIR-Institute of Genomics and Integrative Biology, Delhi; CSIR- Centre for Cellular and Molecular Biology, Hyderabad; DBT- Institute of Life Sciences, Bhubaneshwar; DBT-National Institute of Biomedical Genomics, Kalyani; DBT-InStem-National Centre for Biological Sciences, Bengaluru; National Institute of Mental Health & Neurosciences (NIMHANS), Bengaluru; ICMR- National Institute of Virology, Pune.

It is proposed to continue enhanced genomic surveillance for early detection and containment of the UK variant SARS-CoV-2 strains. However, it is important to understand that like all other RNA viruses, SARS-CoV-2 will continue to mutate. The mutated virus can also be contained by measures like social distancing, hand hygiene, wearing masks and also by an effective vaccine, as and when available.

The Central Government is gearing up for the roll out of COVID19 vaccine across the country. As the vaccine administrators will play an important role in the vaccination process, training of trainers and those who shall administer the vaccine has been taken up across various States.

To strengthen the capacity of our human resource for COVID-19 vaccine introduction and roll-out, detailed training modules have been developed for different categories of vaccine handlers and administrators including medical officers, vaccinators, alternate vaccinators, cold chain handlers, supervisors, data managers, ASHA coordinators and all others involved in the implementation process at different levels. The training includes all operational aspects of training like organization of vaccination sessions, use of Co-WIN IT platform for management of the entire vaccination process,  deployment of HR  Cold chain preparedness, Management of adverse events,  communications and intersectoral coordination, biomedical waste management,  infection prevention protocols etc.

2,360 participants were trained during national level Training of Trainers which comprised state immunization officers, cold chain officers, IEC officials, development partners etc. As on date, the State level trainings had been completed in all States/UTs with participation of more than 7000 district level trainees, except Lakshadweep which will conduct it soon (29th Dec). Cascading down, 681 districts (49,604 trainees) have completed the training of Medical Officers on operational guidelines. Vaccination team trainings have been completed in 1399 out of 17831 blocks/ planning units. It is on going in the other blocks.

To facilitate redressal of queries on COVID-19 vaccination and Co-WIN portal related queries, national 1075 and state 104 Helpline capacity has also been strengthened to address queries beyond their routine support.

As a step to prep up for administration of COVID-19 vaccine, and to assess the readiness of the planned activities, a dry run has been planned in four states to start with, viz. Andhra Pradesh, Assam, Gujarat, Punjab considering the geographical locations. Each State will plan it in two districts and preferably in different (five) session type settings e.g. district hospital, CHC/PHC, urban site, private health facility, rural outreach etc. This will exercise enable end-to-end mobilisation and testing of COVID-19 vaccination process (except the vaccine) and check the usage of Co-WIN in field environment, the linkages between planning, implementation and reporting mechanisms and identify challenges and guide way forward prior to actual implementation including improvements that may be required in the envisaged process. This will also provide a hands-on experience to programme managers at various levels. This two-day activity is planned on 28th & 29th December 2020, and will include activities from the necessary data entries in Co-WIN to vaccine receipt & allocation to deployment of team members, mock drill of session sites with test beneficiaries to reporting and evening meeting. This will also  include testing for cold storage and transportation arrangements for COVID19 vaccine, management of crowd at the session sites with proper physical distancing.

An important focus of the dry run will be on management of any possible adverse events following immunisation (AEFI). In addition, adherence and management of infection control practices at the session site, to prevent disease transmission. The mock drill will include concurrent monitoring and review at the block and district levels, and preparation of feedback to be shared with the State and Union Health Ministry.

Detailed checklist has been prepared by the Union health Ministry and shared with the four States to guide them in the dry run.

The National Expert Group on Vaccine Administration of COVID-19 (NEGVAC) has recommended three prioritised population groups including  Healthcare Workers (HCWs) (about 1 crore), Frontline Workers (FLWs) (about 2 crore), and Prioritized Age Group (about 27 crore).   As vaccines are temperature sensitive and need to be stored in specific temperature, the present cold chain system consisting of 85,634 equipments for storage of vaccine at about 28,947 cold chain points across the country will be used for the cold chain administration. The current cold chain is capable of storing additional quantity of Covid-19 vaccine required for the first 3 Crore prioritised population i.e Health Care Workers and Front Line Workers.

Ministry of Health issued Standard Operating Procedure for Epidemiological Surveillance and Response in the context of new variant of SARS-CoV-2 virus detected in United Kingdom

Posted On: 23 DEC 2020 2:21PM by PIB Delhi

A new variant of SARS- CoV 2 virus [Variant Under Investigation (VUI)-20212/01] has been reported by the Government of United Kingdom (UK) to World Health Organization (WHO) . This variantis estimated by European Center for Disease Control (ECDC) to be more transmissible and affecting younger population. This variant is defined by a set of 17 changes or mutations. One of the most significant is an N501Y mutation in the spike protein that the virus uses to bind to the human ACE2 receptor. Changes in this part of the spike protein may result in the virus becoming more infectious and spreading more easily betweenpeople.

Ministry of Health has issued SOP for Epidemiological Surveillance and Response for the same. The Standard Operating Procedure (SOP) describes the activities to be undertaken at the point of entry and in the community for all International passengers who have travelled from or transited through UK in the past 4 weeks (from 25th November to 23rd December 2020). Any reference to testing in this SOP implies RT-PCR testing only.

The flights from UK are suspended temporarily from 23rd December till 31st December 2020 or till further orders. All the passengers travelling from or transiting through airports in UK and disembarking in India during the intervening period from 21st to 23rd December 2020 would be subjected to RT-PCR test on arrival. In case of a positive sample, it is recommended that spike gene-based RT-PCR test should also be performed. Passengers testing positive will be isolated in an institutional isolation facility in a separate (isolation) unit coordinated by the respective State Health Authorities. Necessary action to send the samples to National Institute of Virology (NIV), Pune or any other appropriate lab for genomic sequencing will be initiated at the facility level. If the genomic sequencing indicates the presence of new variant of SARS-CoV-2 then the patient will be kept in the separate isolation unit and treated as per the clinical protocol.

Those who are found negative on testing with RT-PCR at the airport would be advised quarantine at home. Prior to check-in, the traveler will be explained about this SOP and In-flight announcements must also be made.

Those international travelers from UK who arrived in India since last one month will be contacted by District Surveillance Officers and monitored in community.

State Governments/ Integrated Disease Surveillance Programme will ensure that all the contacts of those travelers from UK or transiting through UK in last four weeks will be traced and monitored. They will also be tested as per protocol and contacts of positive cases would be subjected to institutional quarantine in separate quarantine centers for effective isolation and monitoring.
